This study is looking at whether a digital support tool can help people with Primary care, multimorbidity, Other, or Long-term health conditions, chronic conditions, cardiovascular disease, coronary heart disease, hypertension, heart failure, peripheral arterial disease, chronic kidney disease, stroke, transient ischaemic attack, atrial fibrillation, diabetes mellitus, chronic obstructive pulmonary disease, asthma, epilepsy, depression or severe mental health problems, schizophrenia or psychotic illness, dementia, learning disability, rheumatoid arthritis, frailty. Participants take part in the therapy or support program and complete follow-up questionnaires or assessments. Taking part may give some people access to a digital support tool, but direct benefit is not guaranteed.
